What is Citalopram
Citalopram is an antidepressant prescribed for the treatment of depression of various origins and types, panic disorder and obsessive-compulsive disorder. It is available in tablet form, in doses of 20 mg and 40 mg. The main active ingredient is citalopram, a potent serotonin reuptake inhibitor.
Indications for use of Citalopram
Citalopram is indicated for the treatment of depression of various aetiologies, obsessive-compulsive disorder and panic disorder.
Dosage and method of administration of Citalopram
Tablets should be taken once daily, at any time, regardless of meals. Initial therapy for adult patients: 20 mg once daily. Depending on the severity of the disease, the dose may be increased up to 60 mg daily. Efficacy of the drug is observed after 2-4 weeks. After recovery, Citalopram should be taken for up to six months to prevent relapse. If the patient has recurrent depression, maintenance therapy can be continued for several years to prevent further episodes of the disorder.
In panic disorder: a daily dose of 10 mg once daily is prescribed for adults, after the first week of treatment the daily dose is increased to 20 mg, after which it can be increased to 60 mg. If a person experiences an increase in symptoms of the disorder during initial treatment, the dose is reduced to a minimum. Effective treatment is achieved after 3 months of citalopram therapy.
Obsessive-compulsive disorder: the initial daily dose is 20 mg, which can be increased to 60 mg.
In the elderly: the recommended dose is 20 mg daily, which may be increased to 40 mg daily, depending on the severity of the disease.
In patients with impaired liver function, the dose should be reduced to the lowest daily dose.
Special Indications
There is limited information on experience with the use of citalopram during pregnancy and lactation; the drug tends to pass into breast milk. Drug therapy is not indicated for children under 18 years of age; studies have shown a risk of suicidal behaviour, aggression and anger in children.
Citalopram does not reduce psychomotor activity or intellectual function, but it does reduce attention.
The patient should be closely monitored during treatment, as there is an increased risk of suicidal ideation and self-harm attempts during depression. The risk of suicide may increase in the early stages of recovery, so careful monitoring of the person’s behaviour is necessary.
Contraindications
Do not prescribe treatment in case of hypersensitivity to citalopram or other components of the drug, concomitant MAO inhibitors and within two weeks after discontinuation, during treatment with pimozide, states with signs of serotonergic syndrome.
