What is Clozaril

Clozaril is a neuroleptic that has sedative and antipsychotic effects and does not cause depression. It comes in 25 mg, 50 mg and 100 mg tablets.

Indications for use of Clozaril

It is indicated in patients resistant to standard neuroleptic therapy, in patients at risk of suicidal behaviour and in psychotic disorders of Parkinson’s disease.

Dosage and Administration

The initial dose should be at least 12.5 mg once or twice daily, followed by 25 mg thereafter. If the dose is well tolerated by the patient, the dose is gradually increased to 25-50 mg daily, with the aim of reaching a daily dose of 300 mg in 2-3 weeks. The dose should be increased gradually, once or twice a week. The dose is also gradually reduced. The maximum dose may reach 600 mg daily or 900 mg. Therapy should be continued for at least 6 months.

Geriatric use: start therapy with a low dose of 12.5 mg on the first day, increasing daily doses thereafter up to a maximum of 25 mg.

Contraindications

Hypersensitivity to clozapine, granulocytopenia, agranulocytosis due to chemotherapy, epilepsy, alcoholic and other toxic psychoses, myocarditis, severe cardiac or renal disease, jaundice, liver disease, paralytic ileus, concomitant use with depot neuroleptics, vascular collapse or central nervous system depression.

Side effects

Possible development of eosinophilia, drowsiness, dizziness, headache, nausea, vomiting, development of tremor, akathisia, tachycardia, orthostatic hypotension, syncope, hypertension, development of arrhythmia, myocarditis, constipation, increased body weight, urinary incontinence and urinary retention.
