What is Tegretol
Tegretol is an anticonvulsant, antiepileptic drug. It is also used as a psychotropic medicine, in acute manic states, bipolar affective disorders, in neuralgic diseases (trigeminal neuralgia, neurogenic pain reduction). The main active substance is carbamazepine. It is available in tablet form in doses of 100 mg, 200 mg and 400 mg.
Indications for use
Tegretol is indicated for epilepsy, complex or simple partial seizures, with or without secondary generalisation, bipolar affective disorder, alcohol withdrawal syndrome, idiopathic trigeminal neuralgia, trigeminal neuralgia in multiple sclerosis, idiopathic pharyngeal neuralgia.
Directions for use and posology
The tablets should be taken orally with water. The daily dose is divided into 2-3 doses and should be taken separately from meals.
Treatment of Epilepsy
In adults: The initial daily dose is 100-200 mg once or twice daily. Thereafter, the dose is gradually increased to 800-1200 mg daily. Some patients may require a dose of 1600-2000 mg of Tegretol daily.
Elderly: the dose is adjusted individually and therapy should be started at the lowest dose.
Children: at the age of 5-10 years, a daily dose of 400-600 mg divided into 2-3 doses, 11-15 years – 600-1000 mg divided into 2-5 doses. Calculation should be based on body weight: 10-20 mg per kilogram of body weight. Children over 15 years of age should take the adult dose.
Acute manic states and maintenance therapy of bipolar affective disorder: the dose is usually 400-600 mg per day, but can be increased up to 1600 mg.
Alcohol withdrawal syndrome: the average dose is 200 mg three times a day. In severe syndrome, the drug is prescribed in combination with sedative-hypnotics; once the acute phase is over, Tegretol can be used as monotherapy.
Idiopathic trigeminal and pharyngeal neuralgia: initial dose of 200-400 mg per day, which can be increased to 1600 mg in individual cases.
Contraindications
Tegretol is not indicated in: Hypersensitivity to any of the components, AV block, history of bone marrow suppression, patients with a history of hepatic porphyria, with MAO inhibitors.
Side effects
The most frequent adverse reactions during treatment are leucopenia, thrombocytopenia, agranulocytosis, anaemia, dizziness, headache, nausea, vomiting, stomach pain, diarrhoea, constipation, drowsiness, increased fatigue, insomnia, fever, vasculitis, skin rash, pruritus, urticaria, angioneurotic oedema, decreased appetite, disturbance of accommodation, high blood pressure, dyspnoea, glossitis, stomatitis, pancreatitis, muscle weakness, renal dysfunction, renal failure, colitis.
Special Instructions
Patients should be closely monitored during therapy and a general urinalysis should be performed. It should be noted that the patient may activate a latent psychosis, confusion and anxiety may occur in the elderly.
The drug may cause severe dermatological reactions, so the patient should be warned of the possible risks, and in case of toxic epidermal necrolysis, Lyell’s syndrome, SSD syndrome, hospitalisation should be mandatory.
The drug is ineffective in mild epileptic seizures and myoclonic seizures. In patients with atypical absences, seizures may be exacerbated.
How many times a day
Depending on the prescribed dose, the daily dose is divided into 2-3 doses if the dose is 400-1600 mg per day, or 1-2 doses if the dose is less than 400 mg.
When does Tegretol start to work?
The therapeutic effect occurs between 4 and 24 hours after the first dose. The effects of carbamazepine last 1-3 days. It is excreted in the urine and faeces, the excretion rate decreases in patients with epilepsy and increases in patients under 15 years of age and in the elderly.
How many days should Tegretol be taken?
