What is Zyban

Zyban is indicated for the treatment of depression and to help nicotine-dependent patients stop smoking. It is available as an extended-release tablet with a dose of 150 mg. The main active ingredient, bupropion hydrochloride, is a psychoanalytic.

Its mechanism of action is selective inhibition of norepinephrine and dopamine reuptake, with minimal inhibition of serotonin reuptake. It also acts as a nicotinic receptor antagonist.

Indications

Zyban is used in the treatment of depressive disorders and to facilitate abstinence from tobacco in nicotine-dependent patients.

Dosage and Administration

The tablets should be taken whole, without chewing or crushing, to avoid the risk of cramping. An initial dose of 150 mg once daily should be taken in the morning. If the treatment is not effective, the dose can be increased to 300 mg once daily. The maximum dose of 450 mg daily is prescribed for patients who have not improved after several treatments with a daily dose of 300 mg.

Severe depression requires treatment with bupropion for 6 months or longer after therapeutic effect has been achieved. The dose should be increased and decreased gradually, not more than 100 mg daily. The interval between doses of Zyban should be at least 24 hours.

Contraindications

Zyban should not be used in case of hypersensitivity to any of the ingredients, in children under 18 years of age, in case of seizure disorders, in case of abrupt withdrawal of alcohol or sedatives. Bupropion should not be combined with monoamine oxidase inhibitors and at least two weeks should elapse before discontinuation.

Use During Pregnancy

Overdose

Drowsiness, arrhythmia and unconsciousness have been reported in cases of bupropion overdose. There is no known specific antidote for bupropion. If the dose is exceeded, the patient should be admitted to hospital, airway controlled and ventilated. Activated charcoal may be used and further treatment should be based on severity of condition and symptoms.

Drug Interactions

Caution should be exercised when using bupropion concomitantly with orphenadrine, cyclophosphamide, ifosfamide, ticlopidine and clopidogrel. Treatment should also be initiated when a minimum dose of antiarrhythmics, antipsychotics, antidepressants, selective inhibitors, levodopa, amantadine is reached.
